Request an OT driving assessment or another occupational therapy assessment, and we will be in touch to arrange a time that suits you. Drive & Thrive OT is a mobile service, so assessments are completed in your home and local community across Greater Western Sydney.
How Booking Works
- Complete the booking form below with your details and the assessment you need
- We contact you to confirm the details and answer any questions
- We arrange a time and come to you, in your home or community
- You receive clear recommendations after the assessment
Assessments You Can Request
Choose the assessment that best fits your needs on the form. We offer:
- OT Driving Assessment – fitness to drive, including older drivers and returning to driving
- Functional Capacity Assessment (FCA) – how you manage everyday tasks
- Equipment Needs Assessment – equipment to support safety and independence
- Home Environment Assessment – home set-up, access and safety
- Vehicle Modification Assessment – modifications to support safe driving or transport
What to Have Ready
To help us arrange the right assessment, it helps to have:
- Any referral or request from your GP, specialist or licensing authority
- Relevant medical information or a summary of your condition
- Your licence details, if the request is about driving
- The best contact number and address for a mobile visit

How soon will I hear back?
We aim to respond to booking requests within one to two business days to confirm details and arrange a time.
Do you come to me?
Yes. Drive & Thrive OT is a mobile service, so assessments are arranged in your home and local community across Greater Western Sydney.
Can a GP or support worker book on my behalf?
Yes. Referrers are welcome to submit a booking or referral. If you are a referrer, you can also use our referrals page.
