A simple referral process for assessments across Greater Sydney

We’re glad you’re here. Getting started with Drive & Thrive OT is straightforward simply select the most appropriate referral option below. Once your referral is received, we’ll be in touch to guide you through the next steps and arrange the right assessment.

Choose The Right Referral

Select the assessment and funding option that applies

OT Driving Assessment Referral

For individuals requiring a functional driving assessment completed by an occupational therapist.

Functional Capacity Assessment (FCA) Referral

For assessments supporting NDIS planning, aged care, or service and funding considerations.

Equipment & Environmental Assessment Referral

For equipment needs, home modification, or vehicle access assessments.

Funding options

Referrals can be submitted under:

  • NDIS
  • Self-managed/Plan managed only
  • Private / self-funded
  • Other funders: Workers Compensation, ICare

Clear steps from referral to assessment

Once your referral is submitted, you can expect the following:

Step 1 — Referral received

Your referral is reviewed to confirm the most appropriate assessment type and funding details. This helps ensure the assessment is the right fit from the outset.

Step 2 — Contact and booking

We’ll make contact to discuss availability, location, and any relevant background information. An assessment time is then scheduled at a suitable home or community setting, where appropriate.

Step 3 — Assessment completed

The assessment is completed by a qualified occupational therapist, focusing on real-world function and individual circumstances. Then a clear, professional report is provided to relevant parties as required.
